ABOUT THE ROLE

> Between 4 and 9 sessions/week
> The practice is actively planning to become a training practice, so this could be a plus for a trainer or someone having a special interest in training

ABOUT THE PRACTICE

> Clinical Support Staff: Clinical Pharmacists, Acute Care Nurse practitioner, Practice Nurses, HCAs, Phlebotomists,Care Coordinators. Proactive and embedded ARRS workforce: First Contact Practitioners, Health and Wellbeing Coaches
> Good CQC rating
> High QOF score (between 90% and 95%)
> State of the art purpose-built Health Centre
> Onsite Parking

OTHER RESPONSIBILITIES

> Assessing the health care needs of patients with undifferentiated and undiagnosed problems
> Screening patients for disease risk factors and early signs of illness
> Maintain accurate and up-to-date medical records.
> Accept delegated responsibility for a specific area (or areas) or the QOF
> Conducting consultations with patients to assess and diagnose their medical conditions
> A commitment to life-long learning and audit to ensure evidence-based best practice
> Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
> Participating in training and educational programs.
> Attending training and events organised by the practice or other agencies, where appropriate.
> Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process

REQUIREMENTS

> Newly qualified as well as experienced GPs
> GP Performers list
> Eligibility to practice in the UK independently
> To work collaboratively with the ICB, PCN and Neighbourhood including participation in training events, professional meetings, multi-disciplinary meetings, significant event analysis and other activities in the interest of the individual, team development and improved care of patients
> Experience of continued professional development
> Full registration with GMC
> Ability to motivate patients to improve their health
> Have experience or be comfortable working in a digital environment
> Good clinical skills in assessment, diagnosis and management of acute Primary care presentations
> Suggest improvements for change in a positive and inclusive manner
